Need a Certified Birth Certificate?

If you need a certified record of birth for legal or official use, make sure you request a state-issued certificate. Certificates from hospitals are not valid for legal purposes.

 

What Is a Certified Birth Certificate?

A certified birth certificate is issued by the Kentucky Office of Vital Statistics and includes an official seal. It is valid for all legal and official purposes, such as:

  • Housing applications

  • Obtaining a driver’s license or state ID

  • Employment verification

  • Applying for passports or Social Security

  • School enrollment

  • Insurance and retirement planning

 

What Is Not a Certified Birth Certificate?

Hospital-issued “souvenir” birth certificates do not qualify as certified documents. These cannot be used for any legal or official purpose.

Important: Only state-issued birth certificates are legally valid. Hospital birth records are not accepted for official use.
